Please be advised that in order for Medicare Part B to cover your diabetic supplies (e.g. lancets, blood testing strips, etc.), all diabetic supply prescriptions must meet the following criteria:
Must be written on the original blue prescription paper
Cannot be a fax, e-scrip or phone-in
Must include a diagnosis code
Must specify testing frequency directions (not just "as directed")
