UPDATE--The Donut Hole Rebate
As part of the new healthcare reform package, a $250 rebate is available to Medicare beneficiaries who reach the coverage gap in their 2010 Part D coverage.

There is no application process or forms to fill out to receive this rebate. No one should be contacting the beneficiary to ask for personal information such as a social security number, Medicare number, plan identification number, or bank account information. Attempts by anyone to obtain such information suggest potential fraud and should be   reported immediately.

Part D plan sponsors will notify CMS (Centers for Medicare Services) of beneficiaries who qualify for the rebate. A first round of checks will be sent directly to qualifying beneficiaries from CMS in June, 2010.  A second round of checks will be sent again in September for newly qualifying beneficiaries. CMS will be dependent upon accurate reporting by the plans to make prompt rebate payments. Further plans for later in the year are still to be determined.
